Vaccination and Immunizations Administrative Rules and Regulations
An immunization records must be kept on file for all pupils. By October 15th of each year, a school summary of the immunization status of initial entry pupils must be sent to the Ohio Department of Health.
At a pupil’s initial entry, the parent must present written evidence that the pupil is compliance with the State School Immunization Law. If the parent has not submitted written evidence of compliance, the pupil shall not be entered into school until evidence is submitted. A pupil is in compliance if the pupil
meets one of the following four criteria:
1. The pupil submits written evidence that the pupil meets or exceeds the minimum immunization requirements.
2. The pupil submits a statement, signed by the pupil’s physician, that immunization may be detrimental to the pupil’s health.
3. The pupil submits a statement, signed by the pupil’s parent or guardian; that the parent or guardian objects to immunizations for good cause, including religious convictions.**
4. The pupil submits written evidence that the pupil is “in the process” of completing the required immunizations.
“In the process” (as stated above) means that the pupil is immunized against measles, mumps, rubella, and has had at least one dose of DP/Td vaccine and one dose of polio vaccine.

Measles Measles Vaccine 2* State Law permits a written

Such statement should be closely questioned since many rash illnesses are assumed to be measles, when, in fact they are not.**

Rubella Rubella Vaccine 2* A history of rubella disease is

**NOTE: On August 1, 1992, an updated
Immunization Law went into effect. The
new law requires that
prior to entering the 7th grade, students must have documentation of two doses of live measles,
mumps, and rubella vaccine. The first dose must have been received on or after the first
birthday with a second dose at least 30 days after the first dose. A student who presents
laboratory evidence of immunity to all three diseases shall be exempt from the second dose
requirement.
The requirement is incremental. Thus, by 1993, the requirement will apply to
all seventh and eighth grade students, in 1994, seventh through ninth grade students, etc.
** NOTE: In the event of measles and/or mumps outbreak, only pupils having a record of vaccine or a
physician ( not parent) diagnosed case of measles and/or mumps diseases will be considered
immune.
